Chylous Ascites as Postgastrectomy Complication

Abstract

|
Chylous ascites is uncommon disease that is caused by disruption or obstruction of abdominal lymphatic system. The surgical procedures resulting in chylous ascites include aortic and abdominal aneurysm repair, retroperitoneal lymph node dissection, and inferior vena cava resection. However, chylous ascites is an unusual complication following treatment of gastric carcinoma. Herein, we have reported our experience with chyloperitoneum that occurred after gastrectomy. A
57-year-old man visited our hospital for postprandial abdominal pain and dysphasia. Two years ago, he underwent distal gastrectomy with D2 lymph node dissection for T4aN1M0 gastric cancer. Under the impression of recurred gastric cancer, we performed completion total gastrectomy and transverse colectomy. On the third postoperative day, chylous ascites was diagnosed. During conservative management. intestinal obstruction was found and adhesiolysis and bypass surgery were done. Onthe 127th postoperative day, patient died in multiple entero-cutaneous fistula and sepsis.
